I agree to the FAITH HAVEN RESCUE adoption process, and agree to a possible home visit.    I understand FAITH HAVEN RESCUE may check my veterinary reference.  I understand there is an adoption donation associated with adoption of a pet from FAITH HAVEN RESCUE  and that the donation  is tax deductible according to IRS 501(c)3 guidelines.  I understand this donation will ensure the organization is equipped to rescue another homeless pet or provide for current rescues as they await their forever homes.  I understand that if I no longer want or can no longer care for my adopted dog, I will notify FAITH HAVEN RESCUE by email and provide a 7 day period to allow FAITH HAVEN  to make arrangents for my pet to be taken back into rescue.   I agree to indemnify and hold harmless FAITH HAVEN RESCUE against any losses, lawsuits, claims, injury, damages incurred by me or to any persons or property by my adopted dog, once adoption has been completed.  I understand that FAITH HAVEN RESCUE will disclose any of the dog's health or behavior issues known by the above named rescue group before adoption is completed. I understand that if I no longer want my dog, or am no longer able to care for my adopted dog, I will be directed to surrender my pet to FAITH HAVEN RESCUE.  By clicking the submit button, I verify all of the above information is true and accurate.
